Please provide the names and contact information for a minimum of one personal reference who has knowledge about your experience with animals, in particular, with dogs or cats
*Volunteers do not receive payment for their time or services
*If I accept an assignment, I promise to complete that assignment as required, and if unable will give MLAR appropriate notice to find a replacement volunteer
*If that assignment involves the use of my vehicle, MLAR is not responsible for any damages from that assignment..ie…tears, scratches, dirt, etc.
*When transporting, I agree to follow the directions and protocols of MLAR
*Volunteers are not to expose minors under the age of 18 to the rescue animals
*Volunteer status may be revoked at any time at the discretion of MLAR
My signature confirms that I have read and agree to the terms and conditions – rules and guidelines of MLAR. By signing this application…I understand that some rescues come from bad or abusive situations and may have behavior issues or exhibit aggression. The rescues may require special handling or training. MLAR may get minimal information on the animals we rescue. Until fully assessed, we cannot guarantee the temperament. Therefore, I will not hold MLAR responsible for any damage, injury, or harm caused directly or indirectly to any person or property by any animal I may decide to volunteer my time to help. I agree that I will not expose any MLAR rescue animals to children under the age of 18. If indeed a child is involved, MLAR will not be held responsible for any incident that may occur. Volunteer applicants understand that MLAR reserves the right to refuse any applicant at their discretion.
